Overview
The Puyallup School District Health Services team believes that optimal health is essential for maximum utilization of educational opportunities; therefore, the health status of the child bears a direct relationship to the quality of his or her educational achievement.
We provide a student-centered, collaborative approach to learning and health in partnership with students' families and the school community that will propel us towards our goal of supporting student health and academic success by contributing to a healthy and safe school environment.
When Is My Child Too Sick For School?
There may be times when your child is too sick to attend school.
The information below courtesy of the Tacoma-Pierce County Health Department provides useful information to consider when deciding whether your child is too sick to attend school and when to contact a healthcare provider.
Immunization Information and Forms
Washington state law requires all students attending public schools to be immunized against diphtheria, tetanus, pertussis (whooping cough), polio, measles, mumps, hepatitis B, varicella and rubella before registering for school. Required Immunizations for 2024-25 School Year
Preschool (Age 19 months to <4 years on September 1st)
- DTaP/Tdap (Diphtheria, Tetanus, Pertussis) - 4 doses DTaP*
- Hepatitis B - 3 doses
- Hib (Haemophilus influenzae type B) - 3 or 4 doses* (depending on vaccine)
- MMR (Measles, Mumps, Rubella) - 1 dose
- PCV (Pneumococcal Conjugate) - 4 doses*
- Polio - 3 doses*
- Varicella (Chickenpox) - 1 dose**
Preschool/Transitional Kindergarten (4 years of age or older on September 1st)
- DTaP/Tdap (Diphtheria, Tetanus, Pertussis) - 5 doses DTaP*
- Hepatitis B - 3 doses
- Hib (Haemophilus influenzae type B) - 3 or 4 doses* (depending on vaccine; Not required at 5 years of age or older)
- MMR (Measles, Mumps, Rubella) - 2 doses
- PCV (Pneumococcal Conjugate) - 4 doses* (Not required at 5 years of age or older)
- Polio - 4 doses*
- Varicella (Chickenpox) - 2 doses**
Kindergarten – 6th Grade
- DTaP/Tdap (Diphtheria, Tetanus, Pertussis) - 5 doses DTaP*
- Hepatitis B - 3 doses
- MMR (Measles, Mumps, Rubella) - 2 doses
- Polio - 4 doses*
- Varicella (Chickenpox) - 2 doses**
7th -11th Grade
- DTaP/Tdap (Diphtheria, Tetanus, Pertussis) - 5 doses DTaP*; Plus Tdap at age >10 years
- Hepatitis B - 3 doses
- MMR (Measles, Mumps, Rubella) - 2 doses
- Polio - 4 doses*
- Varicella (Chickenpox) - 2 doses**
12th Grade
- DTaP/Tdap (Diphtheria, Tetanus, Pertussis) - 5 doses DTaP*; Plus Tdap at age >7 years
- Hepatitis B - 3 doses
- MMR (Measles, Mumps, Rubella) - 2 doses
- Polio - 4 doses*
- Varicella (Chickenpox) - 2 doses**
*Vaccine doses may be acceptable with fewer than listed depending on when they were given.
**Health care provider verification of history of chickenpox disease is also acceptable. Students must get vaccine doses at the correct timeframes to be in compliance with school requirements.
